Samuel “Eric” Storm Memorial Scholarship

Eligibility

• Be a U.S. citizen.
• Have lived in Mississippi for the previous five years.
• Be a graduate student pursuing an advanced degree in Physical Therapy, Occupational Therapy, or Speech Therapy.
• Be accepted to or currently enrolled in an accredited graduate program.
• Intend to become a licensed Physical Therapist, Occupational Therapist, or Speech Therapist.
• Students attending eligible programs outside Mississippi may apply.

Requirements

• Complete the online scholarship application.
• Provide required supporting documentation.
• Submit two letters of recommendation.
• Provide a statement explaining why you selected your field of study.
• Submit a 500-word essay responding to one of the published prompts.

How to Apply

3. 3Obtain two recommendation letters based on relevant observation, shadowing, or similar experience.
4. 4Write the required 500-word essay.
