Oak Hills Healthcare Scholarship Program

Eligibility

• Be an employee of Oak Hills Living Center.
• Have worked at Oak Hills Living Center for at least three months.
• Work an average of at least 10 hours per week at Oak Hills Living Center.
• Maintain a block or set work schedule sufficient to meet the 10-hour-per-week requirement; casual employees are not eligible.
• Maintain at least a 2.50 GPA while participating in the scholarship program.
• Pursue education that advances a career within Oak Hills Living Center or the long-term care field.
• Attend an eligible education or training program; out-of-state college tuition may be eligible as long as the employee continues working at the Minnesota facility.
• Remain in good employment standing with Oak Hills Living Center.

Requirements

• Provide an itemized tuition bill each quarter or semester.
• Provide a current class schedule each quarter or semester.
• Provide a copy of grades each quarter or semester.
• Maintain at least a 2.50 GPA.
• Continue averaging at least 10 hours of work per week at Oak Hills Living Center.
• Use scholarship funding only for eligible education-related expenses.

How to Apply

4. 4Apply for the Oak Hills Healthcare Scholarship Program through Oak Hills Living Center.

6. 6Submit grades each quarter or semester while receiving assistance.
7. 7Maintain the required 2.50 GPA and employment eligibility throughout participation.
