Eligibility
- Be an employee of Chautauqua Nursing and Rehab Center or the spouse, son, or daughter of an employee.
- If no eligible center applicants apply, northern Chautauqua County residents entering or continuing health care education may be considered.
Requirements
- Complete the form and write an essay of 500 words or fewer on your health care goals and reasons for pursuing the field.
