Eligibility

• Be a high school senior pursuing a career in health care.
• Be a resident of Madelia, Lake Crystal, or the surrounding area.
• Be accepted into an accredited associate or bachelor's degree program.
• Actively volunteer or help others.

Requirements

• Complete the Madelia Health Foundation scholarship application.
• Submit academic verification, a transcript, or other required academic documentation.
• Submit two letters of recommendation.
• Submit the required typed essay.

How to Apply
