Eligibility

  • Be a child, stepchild, grandchild, spouse, domestic partner, or primary caregiver of a person with ALS.
  • Be a graduate of a Maine high school or Maine GED program.
  • Attend or plan to attend a postsecondary educational institution.
  • Eligible institutions include four-year colleges and two-year associate degree programs.

Requirements

  • Complete the Joseph W. Mayo ALS Scholarship application.
  • Provide documentation establishing the qualifying relationship to a person with ALS.
  • Provide proof of graduation from a Maine high school or GED program.
  • Provide current or planned postsecondary enrollment information.
  • Submit all required supporting documentation.
