Requirements

  • Submit the fillable application, official PT/PTA transcript, headshot, publicity release and department-chair credit verification.
  • Include activity/honor information and an original nominee statement; the event page limits it to 1,500 words or 9,000 characters.
  • Confirm the deadline: sponsor procedure says October 31, while cycle/event pages say November 1, 2026. Aim to submit by October 31.
  • Opt into Sunshine consideration and answer its additional pediatric-care and service prompts, up to 1,500 words or 9,000 characters under current event instructions.
