Dick Mills Legacy Scholarship

Eligibility

• Be a graduating student at a high school or in an approved virtual high school environment.
• Be from the Lake City, Minnesota market.
• Be a Wisconsin or Minnesota resident.
• Have a minimum cumulative GPA of 3.0 on a 4.0 scale or equivalent.
• Plan to enroll full-time as an undergraduate student at an accredited school, college, or university in Wisconsin or Minnesota during the next succeeding term.
• Complete one full semester and show proof of registration for a following semester, or be registered to graduate, before scholarship payment is made.

Requirements

• Complete the Dick Mills Legacy Scholarship application packet.
• Provide the academic and personal information requested by Forward Bank.
• Submit all required materials by the annual deadline.
• If selected, complete one full semester.
• Provide proof of registration for the following semester or proof that you are registered to graduate before payment is released.
• Complete the Scholarship Disbursement Request Form provided to recipients.

How to Apply

1. 1Confirm that you meet the Lake City market, residency, graduation, and 3.0 GPA requirements.
2. 2Complete the Dick Mills Legacy Scholarship application packet.
3. 3Provide all information and supporting materials requested by Forward Bank.
4. 4Submit the complete application by the annual deadline.
5. 5If selected, enroll full-time at an accredited Wisconsin or Minnesota institution.
6. 6Successfully complete one full semester.
7. 7Provide proof of registration for the following semester or upcoming graduation.
8. 8Submit the required Scholarship Disbursement Request Form to receive the award.
