Care Providers of Minnesota Foundation Scholarship

Eligibility

• Be currently employed full-time or part-time by a facility, housing organization, home care organization, or community member of Care Providers of Minnesota.
• Have at least one year of long-term care work experience.
• Be enrolled, or become enrolled no later than fall 2026, in an accredited course of study leading to an eligible career path.
• The course of study must support a career serving seniors and/or persons with disabilities, assisted living director licensure, or a career in post-acute care, long-term care, or long-term services and supports.
• Eligible professional certification programs may also qualify.
• Remain employed by a qualifying Care Providers of Minnesota employer at the time of scholarship selection and when the award is distributed.

Requirements

• Complete the Care Providers of Minnesota Foundation scholarship application.
• Submit responses to the required essay questions.
• Submit a recommendation from an administrator, executive director, or direct supervisor in long-term care.
• Submit a personal or coworker recommendation.
• Provide proof of enrollment before scholarship funds are awarded.

How to Apply

1. 1Confirm that your employer is a Care Providers of Minnesota member organization.
2. 2Confirm that you have at least one year of long-term care work experience.
3. 3Confirm that your education or professional certification supports an eligible long-term care or services career.
4. 4Complete the online scholarship application.
5. 5Answer all required essay questions.
6. 6Obtain the required administrator, executive director, or supervisor recommendation.
7. 7Obtain the required personal or coworker recommendation.
8. 8Submit the completed application by July 10.
9. 9If selected, provide proof of enrollment before receiving scholarship funds.
